Do I need a referral or prescription from a MD?

The state of IL does not require a doctor’s referral or a prescription for physical therapy. If you have a prescription, please bring it with you so I can contact your physician if any issues arise or you do not demonstrate appropriate progress.

Medicare requires a Certified Plan of Care within 30 days of the initial visit. As of now, I am not a certified Medicare Provider.

Do you accept my insurance?

I am in-network with BCBS PPO and BCBS Choice plans. I am in-network with BCBS HMO and Anthem BCBS plans but a payment of $160 is required on the date of service. Your claim will be electronically submitted for you. If BCBS reimburses for your care, I will reimburse any received amount to you.

What if I have Medicare?

I am not accepting Medicare at this time. If you are on Medicare, I can provide wellness treatment or dry needling treatment (if you are an established patient) for cash pay.

Can I be treated for physical therapy if SPECIALIZED physical therapy isn’t in network with my insurance?

Yes, a payment of $160 is due at the end of your treatment session. I will provide you with an electronic invoice or “superbill” with the appropriate codes within 48 hours of your appointment. You will be able to download the document and submit it to your insurance company. Before you schedule your physical therapy appointment, please contact your insurance company and inquire about reimbursement for ”out-of-network physical therapy” expenses sent in via self-claims.

Why don’t you take my insurance?

I am a solo practitioner with low volume. Billing clearinghouses won’t work with low-volume clinics such as SPECIALIZED Physical Therapy. Insurance companies have made reimbursements extremely difficult. I want to focus my time on your health care, not reimbursement.

What is a dry needling visit?

Trigger point dry needling (TDN) involves placing a thin filament needle into the muscle at the trigger point. The intent of TDN is to elicit a local twitch response to normalize muscle tone. These visits are designed for patients who have already been evaluated and treated by me within the past 2-4 months for the affected body part. Dry needling is the only service provided during this session.

What is boutique style therapy?

SPECIALIZED Physical Therapy models its practice on providing attentive, personalized treatment. Sessions are a true one-on-one appointment where the attention is on you and only you.

Do you have parking?

There is no designated parking. Metered street parking is available on Armitage Ave. Parking is free on Racine Ave before 6:00 pm.

How are you handling COVID-19 precautions?

Masking is optional since we are in an exercise facility. If you feel you are at risk or may be contagious, please wear a mask and I will
wear a mask if requested or if I feel I am at risk.

How do I cancel my appointment?

Cancellations made with more than 24 hours notice can be done through Janeapp. To cancel an appointment with less than 24 hours notice, please send a message with the date and time of your appointment to maureen@specializedpt.net. Cancellation fees apply:

  • No Show - $75.00 cancellation fee if you do not show up for your appointment

  • Less than 24 hours - $75.00 cancellation fee if an appointment is canceled with less than 24 hours notice (emergency and special circumstances are considered).

  • The full $160 treatment fee will be applied if another cancellation occurs after the $75 fee has been applied.
