Hour long session of physical training, exercise, education, instruction and consulting regarding wellness and health techniques.
Services do not include physical therapy services as the term is defined under the Illinois physical therapy practice act and its regulations.

Dry Needling
20-25 min / $85
Half-hour long session using a thin filament needle to penetrate the skin and underlying trigger points for the management of neuromuscular pain and movement impairments.
THIS IS ONLY FOR ESTABLISHED PATIENTS WHO HAVE BEEN EVALUATED OR TREATED IN THE PAST SIX MONTHS FOR THE AFFECTED BODY REGION.

In-Network provider for BCBS PPO and BCBS Choice plans (Not BCBS HMO or Anthem BCBS plans) Claims will be electronically submitted for you. If there is a co-payment or deductible charge, you will be notified of your balance and your account will be charged.
BCBS HMO or ANTHEM plans:
I am In-Network for these plans. A payment of $160 is due at the end of your treatment session. Your claim will be electronically submitted for you. If BCBS reimburses for your care, I will reimburse any received amount to you.
Out-of-Network provider for patients with out-of-network benefits
A payment of $160 is due at the end of your treatment session. I will provide you with an electronic invoice or “superbill” with the appropriate codes within 48 hours of your appointment. You can download the document and submit it to your insurance company. If you have out-of-network benefits and you have met your deductible, your insurance company will reimburse you directly. Prior to your first visit, please call your insurance company and inquire about your physical therapy “out-of-network” benefits.
Medicare
I am not accepting Medicare at this time. I am able to provide wellness and dry needling treatments for this population.

Standard flat rate of $160 apply for out-of-network plans and for wellness visits. This is due at time of service.
Rate of $85 for a dry needling session. This is due at time of service.

In order to ensure all available appointment slots are filled, the following cancellation fees will be applied:
No Show - $160.00 no show fee
Less than 24 hours - $85.00 cancellation fee (emergency and special circumstances are considered).
The full $160 treatment fee will be applied if multiple cancellations occur.
